Community Pharmacy Safety Initiative

ISMP Canada has contracted with the Ontario Ministry of Health and Long Term Care to expand medication safety activities in community pharmacies.

This initiative for 2004/5 includes:

• Implementation of the community safety medication self-assessment tool
• Coordinate the distribution of a medication safety alert to community pharmacies.

Action to date includes:

• Introduction of the tool to the Ontario College of Pharmacists staff and to corporate representatives of select pharmacy chains.
• Working with these representatives to generate uptake of the tool
• The safety bulletins are being distributed to interested community pharmacists. If interested please contact Kris Wichman.
