Ontario Registration Form

With support from the Ontario Ministry of Health and Long-Term Care, the Institute for Safe Medication Practices Canada is pleased to offer another patient safety tool to hospitals in Ontario.

Analyze-ERR® is a medication error reporting and analysis software program which allows hospitals to input adverse drug events and underlying contributing factors. It creates a local database of medication incidents for retrieval, follow-up reports and analysis.

Analyze-ERR® will assist your safe medication practice committee and risk management team to ensure a proactive approach to medication safety and to facilitate a culture of safety.

Between 2002 and 2003 ISMP Canada conducted a pilot system analysis study of medication errors. Thirteen Ontario hospitals, using the Analyze-ERR® program, participated in the study. The latest version of Analyze-ERR® was designed with input from these hospitals.

Complimentary use of Analyze-ERR® is offered to Ontario hospitals. Hospitals interested in using the software can register using the form on this page. Training sessions will be conducted, with specific locations and dates provided to registered and participating hospitals.

Hospitals can choose to submit selected incident and near-miss data to ISMP Canada in order that data from across the province and Canada can be aggregated. Participating hospitals will have access to the aggregated data with various search capabilities.
