Canadian Medication Incident Reporting and Prevention System (CMIRPS)

The key partners in the development and implementation of CMIRPS are ISMP Canada, Canadian Institute for Health Information (CIHI), and Health Canada.

Background Information on the CMIRPS

In the fall of 2000, an invitational workshop was co-hosted by the Canadian Society of Hospital Pharmacists (CSHP) and Health Canada’s Bureau of Licensed Product Assessment (BLPA) to address a number of key questions related to medication incident reporting and prevention. The workshop led to the creation of a national coalition of stakeholders – the Canadian Coalition on Medication Incident Reporting and Prevention (CCMIRP). This coalition led the creation of a business plan for the development and implementation of a national medication incident reporting and prevention system.

In the fall of 2003, funding for the Canadian Medication Incident Reporting and Prevention System (CMIRPS) was secured. Funding for the CMIRPS is being made available through the signing of Contribution Agreements between Health Canada and CIHI, and between Health Canada and ISMP Canada.

Purpose of the CMIRPS

The purposes of the CMIRPS program are to:

  • Coordinate the capture, analysis and dissemination of information on medication incidents;
  • Enhance the safety of the medication use system for Canadians; and
  • Support the effective use of resources through the reduction of potential or actual harm caused by preventable medication incidents.

The goals of the CMIRPS information system are to:

  • Collect data on medication incidents;
  • Facilitate the implementation of reporting of medication incidents;
  • Facilitate the development and dissemination of timely, targeted information designed to reduce the risk of medication incidents (e.g. ISMP Canada Safety Bulletins); and
  • Facilitate the development and dissemination of information on best practices in safe medication use systems.
ISMP Canada has made a variety of reporting mechanisms available to individual practitioners, including telephone or web portal electronic submission. Reports may be submitted anonymously if preferred.
