Root Cause Analysis (RCA) and
Failure Mode and Effects Analysis (FMEA) Workshops


ISMP Canada, with support from Health Canada is hosting an RCA and an FMEA workshop for healthcare practitioners in the acute care, long term care, and community care sectors. The workshop will be held in Moncton, New Brunswick on February 18 and 19, 2010.

Registration:
Cost is $265 per person/day to attend a workshop. A reduced rate of $475 will be offered to those who wish to attend both days. All costs include GST.
Attire:
Business casual dress is suggested. You may wish to dress in layers as the room temperature may vary.
Workshop Material:
All workshop material, including the Canadian RCA Framework© and Canadian FMEA Framework© will be provided onsite


RCA workshop will include:

  • An overview of the systems approach.
  • RCA framework presentation.
  • Practice exercises using the RCA framework.
  • Copy of the Canadian RCA Framework©.


FMEA workshop will include:
  • An overview of the systems approach.
  • FMEA framework presentation.
  • Practice exercises using the FMEA framework.
  • Copy of the Canadian FMEA Framework©.

The workshop is designed for those working in healthcare in both clinical and non-clinical areas, including: Directors of Care, Risk Management Officers, Patient Safety Officers, Physicians, Nurses, Pharmacy Directors, Pharmacists, Pharmacy Technicians, Staff Educators, Quality Managers, Allied Healthcare Professionals, Food Services Personnel, Maintenance Staff and others who want to be involved in initiatives to improve patient safety.

Root Cause Analysis (RCA) Workshop
ISMP Canada in collaboration with the Canadian Patient Safety Institute and Saskatchewan Health, developed the Canadian Root Cause Analysis Framework© for analysis of critical incidents in healthcare and a one day RCA workshop for healthcare facilities. RCA is a tool to help investigate adverse events, identify and analyze root causes and contributing factors, and develop recommendations. It is a team-based, multidisciplinary approach that includes frontline staff.

Failure Mode and Effects Analysis (FMEA) Workshop
ISMP Canada has developed the Canadian Failure Mode and Effects Analysis Framework© as a team-based, systematic and proactive approach identifying ways a process or design can fail, why it might fail, the effects of the failure and how it can be made safer. At the end of the one day FMEA workshop participants will increase their knowledge about the impact of system factors on error potential, understand how human factors engineering principles apply to healthcare environments, learn the steps to conduct an FMEA and develop the expertise to serve on an FMEA team in a healthcare setting.

  1. Who should attend the workshop?
    The workshop is open to all those working in healthcare in both clinical and non-clinical areas. Participants may include, but are not limited to: CEOs and Home Administrators, Directors of Care, Chief Nursing Officers, Quality/Risk Managers, Pharmacy Directors, Physicians, Staff Educators, Nurses, Pharmacists, Pharmacy Technicians, Allied Healthcare Professionals, Food Services Personnel, Maintenance Staff and others who want to be involved in initiatives to improve patient safety.
  2. How much is one workshop
    The workshop fee is $265 per participant for one day including GST.
  3. Can I attend both workshops?
    Yes participants are invited to attend for two days at a reduced rate of $475 including GST. Space is limited to 30 people per day so register early.
  4. Is there a discount if more than five individuals from the same institution want to attend?
    Yes, a reduced rate is available to facilities that send five or more individuals. Multiple registrations must be submitted together to qualify for the reduced rate. For more information email education@ismp-canada.org
  5. How do I register more than one participant from our organization?
    Fill out a separate registration form for each individual and mail or fax with payment to ISMP Canada. Provide one cheque or credit card payment for all registrants from the same organization.
  6. How do I pay for the workshop?
    Payment can be made by cheque or credit card (Visa and MasterCard only) or PayPal. All payments by cheque must be mailed with the registration forms. To pay by credit card, complete the electronic registration form, or print and fax the pdf registration form.
  7. Will I receive a receipt for payment?
    Yes, ISMP Canada will issue a receipt by email once payment is received in full.
  8. How will I know that my registration has been accepted?
    You will receive a confirmation email from ISMP Canada within 10 days of receipt of payment, indicating that your registration has been processed and payment received.
  9. What time should I arrive?
    8:00 am sharp.
