The workshops and webinar are designed to provide healthcare providers working in the community with an overview of medication reconciliation. This will include a focus on the Best Possible Medication History (BPMH) and a review of how medication reconciliation is conducted in various practice settings. Participants will be encouraged to share and learn from each other's experiences and will have an opportunity to inform the development of a medication reconciliation best practices network.

Learning Objectives:

By the end of this workshop, participants will be able to:

  • Understand patient/family/caregiver perspectives about communicating their medication information as they transition into the community setting
  • Explain the role of medication reconciliation and how it improves patient care
  • Describe the process of completing a BPMH
  • Understand medication communication issues facing health care providers in various practice settings



Audience:
Healthcare providers working in the community including community pharmacists, team pharmacists, nurses, clinicians, quality improvement specialists working in primary care; members of Health Link teams

Speakers

Lynn Riley, RN Medication Safety Specialist and Educator
Lynn has more than 30 years of clinical, teaching and managerial nursing experience. She has a varied background that includes staff nurse in critical care, outpatient clinic and homecare. Lynn has also held positions in the Quality and Patient Safety department at Sunnybrook Health Sciences Centre in Toronto and as a Safety and Improvement Advisor with Safer Healthcare Now! in Atlantic Canada.

Currently, Lynn is a homecare nurse in her community and is a Medication Safety Specialist and Educator at ISMP Canada.


Alice Watt, RPh, Medication Safety Specialist
Alice joined ISMP Canada in 2007 as a Medication Safety Specialist and received her BSc. (Pharm.) from the University of British Columbia. She is actively practicing in a community hospital as a clinical pharmacist and has over 15 years of experience in community and acute care settings.

Alice is currently involved in incident analysis, newsletter development and engaging consumers and health care practitioners in safe medication practices. In the past years, she has worked on pediatric opioid safety, the WHO High Fives: Action on Patient Safety Assuring Medication Accuracy at Transitions in care, and supported Safer HealthCare Now! medication reconciliation quality improvement initiatives.

Background

Medication Reconciliation is a formal process in which healthcare providers partner with patients and their families to ensure accurate and complete medication information transfer at interfaces of care. This includes admission and discharge from a hospital or changes in care setting, service, or level of care.
