Application of TALLman Lettering for Selected High Alert Drugs in Canada

Drug name confusion, particularly because of look-alike, sound-alike (LASA) name attributes can be a contributing factor to medication related adverse events. TALLman lettering is one of many risk mitigation strategies to minimize errors involving LASA drug names.

ISMP Canada invites healthcare professionals to participate in a one hour complimentary webinar that will provide an opportunity for participants to learn more about the development of a list of "TALLman Lettering for Look-alike/Sound-Alike Drug Names in Canada".

Learning Objectives:

At the end of the webinar, participants will:

  • Understand the methodology used to apply TALLman lettering to selected LASA drug name pairs of concern in Canada.
  • Learn about the principles for applying TALLman lettering in Canada.
  • Consider alternative and complementary strategies that can be used to reduce the risk of errors associated with look-alike, sound-alike (LASA) drug names.

Audience:
Healthcare professionals involved in procurement, prescribing, dispensing, transcribing and administering look-alike, sound alike drugs; pharmacy managers who oversee patient safety activities, clinical informatics personnel involved in the development or implementation of IT medication management systems; administrators; quality and risk management personnel and those interested in patient safety in general.

Speaker:

Kim Streitenberger, RN, Project Lead - Institute for Safe Medication Practices Canada
Kim joined ISMP Canada first in 2007 as a patient safety consultant with a focus on medication reconciliation. She is a registered nurse with more than 34 years of clinical, quality improvement and patient safety experience. Kim’s clinical background is in pediatrics at Laurentian Hospital in Sudbury, Ontario and pediatric critical care at the Hospital for Sick Children. She has held formal program and corporate level quality improvement, patient safety and risk management roles. Kim has a certificate in Quality Improvement and Patient Safety from the University of Toronto. She was a recipient of the OHA Health Achieve Patient Safety Award in 2005 and the Baxter Guardian Scholarship for Excellence in Patient Safety in 2010 for work on improving communication during patient transitions.
Kim returned to ISMP Canada in 2014 and is currently a Project Lead with a focus on health product labelling and packaging and medication reconciliation. She has been a frequent speaker at local, national and international conferences and has coauthored a variety of clinical and patient safety related articles.
