BPMH Training for Pharmacy TechniciansUnderstanding the hospital pharmacy technician’s role in the medication reconciliation process
Thursday, February 12, 2015 This one-day workshop is designed to provide hospital pharmacy technicians with an overview of medication reconciliation. There will be a specific focus on the importance of the Best Possible Medication History (BPMH) within the medication reconciliation process and participants will learn how to conduct a BPMH interview and document the necessary components of a BPMH. At the completion of this workshop, participants will be able to:

Thursday, February 12, 2015 Kimindra Tiwana, RPh, BSc. Pharm, ACPR Kim has been a practicing pharmacist since 2001. She received her pharmacy degree from Dalhousie University, College of Pharmacy and then went on to do a residency with the Queen Elizabeth Health Sciences Centre. The majority of Kimindra’s clinical experience comes from working on general internal medicine and cardiology at University Health Network - Toronto Western Hospital. In 2008 she joined Mount Sinai’s Informatics team and was involved with the implementation of the computerized physician order entry system throughout the hospital. In 2010, Kimindra joined the Institute for Safe Medication Practices Canada as a medication safety specialist. Her focus at ISMP Canada has mainly been on medication reconciliation initiatives, including the development of iPhone app for patients to store and communicate their medication related information.
