Incident Analysis Framework: Train-the-Trainer Workshop

The Institute for Safe Medication Practices Canada (ISMP Canada) and the Canadian Patient Safety Institute (CPSI) have partnered to offer an Incident Analysis Framework train-the-trainer workshop that utilizes the models of PSEP – Canada, and the training and analysis expertise from ISMP Canada. It is based on the Canadian Incident Analysis Framework.

This one-day workshop is designed for PSEP – Canada Trainers. It will provide them with the tools to teach the Canadian Incident Analysis Framework in their organizations. The Framework enhances patient safety by supporting organizational learning, quality improvement and improving the success of incident analysis.

Learning Objectives:

At the completion of this workshop, participants will be able to:

  1. understand the key components of the Framework,
  2. understand the analysis process including the different methodologies, and developing and managing recommended actions, and
  3. be able to use the Framework to support organizational learning, quality improvement, and improve the success of incident analysis in enhancing patient safety.

This workshop will be conducted by Donna Walsh RN, BScN, FISMPC and Kate Wilkinson RN, BScN, MSc Health Administration

Donna Walsh, RN, BScN, FISMPC

Donna Walsh joined ISMP Canada in 2005. At the end of the following year, she completed the 2006 ISMP Canada, Fellowship in Safe Medication Management. As Education Lead, her responsibilities include: development, co-ordination and implementation of medication safety education initiatives.

Donna has worked and taught in a number of academic and healthcare settings. As part of her work at ISMP Canada, she has facilitated Root Cause Analysis, Failure Mode and Effects Analysis, and Medication Reconciliation/Best Possible Medication History (BPMH) workshops and medication safety presentations and webinars. Donna is a co-author of the revised Canadian Incident Analysis Framework released in 2012; it was previously known as the Canadian Root Cause Analysis Framework, 2006.


 
 

Kate Wilkinson RN, BScN, MSc Health Administration

Kate is a Registered Nurse and is the Associate Vice President, Patient Experience at Markham Stouffville Hospital in Markham, Ontario. In this role, Kate is excited to lead the hospital’s strategy to deliver all programs with a patient -and family -centered care focus.

Developing true partnerships with patients and promoting a greater degree of patient and family involvement are important in building a culture which will achieve the mission of providing an exceptional patient experience. Essential to achieving this experience is to ensure high quality, safe care to every patient  - everyday. Kate has been an advocate for the Patient Safety Education Program (PSEP) from the beginning and its role in building staff and leadership capability to achieving safe patient care.

The Institute for Safe Medication Practices Canada (ISMP Canada) and the Canadian Patient Safety Institute (CPSI) have partnered to offer an Incident Analysis Framework train-the-trainer workshop that utilizes the models of PSEP – Canada, and the training and analysis expertise from ISMP Canada. It is based on the Canadian Incident Analysis Framework (the Framework). The Framework is a resource to support those responsible for managing, analyzing, and learning from patient safety incidents to determine what happened, how and why it happened, what can be done to reduce the risk of recurrence, and what can be learned.

This workshop will offer PSEP – Canada trainers resources and training methods to teach others the concepts and best practices outlined in the Canadian Incident Analysis Framework. Participants will be provided with a copy of the Framework, and will learn about the PSEP – Canada module on Incident Analysis that was released in 2014. A joint certificate from CPSI and ISMP Canada will be offered to individuals who attend the program.

Please note, the Incident Analysis Framework workshops will only be open to those who have completed the PSEP – Canada training.
