Multi-Incident Analysis Workshop
Analyzing medication incidents one group at a time

1 day workshop

This workshop has been assigned 6.5 CEUs by the Ontario College of Pharmacists. This program is supported by the Canadian Medication Incident Reporting and Prevention System (CMIRPs).

This one day interactive workshop presents the key concepts for conducting a multi-incident analysis (aggregate) analysis, a technique designed to maximize efficiency by analyzing a group of medication incidents sharing a common topic.

During this interactive and hands-on workshop you will learn a step-wise process of conducting a multi-incident analysis, and situations where it is most beneficial. You will have the opportunity for hands-on practice under the guidance of ISMP Canada experts. A variety of take home materials and tools will be provided, enabling you to confidently analyze your facility's medication incidents upon return to your home facility.

Learning Objectives: At the completion of this workshop, participants will be able to:

  1. Describe how multi-incident analysis complements other medication incident analysis strategies
  2. Describe the steps of a multi-incident analysis
  3. Apply the step-wise multi-incident analysis approach to begin conducting analysis of small groups of medication incidents

This workshop is complementary to the new “Going beyond the numbers” workshop, which will be offered on Sept. 17, 2015. Participants are encouraged to attend both, and will be entitled to a discount of 15% off the cost of the new workshop on Sept. 17th. Roger Cheng, RPh, BScPhm, PharmD

Roger joined ISMP Canada in 2006 as an analyst. His current responsibilities as a project leader include conducting individual and multi-incident (aggregate) analysis from the medication incidents in the ISMP Canada Medication incident database.

Roger holds a Bachelor of Science in Pharmacy degree (1999) and a Doctor of Pharmacy degree (2003) from the University of Toronto. Roger has worked in both community and hospital pharmacy settings, including staff pharmacist positions at Princess Margaret Hospital and York Central Hospital.
