February 5, 2015: Methadone Medication Incidents in Community Pharmacy

ISMP Canada’s Community Pharmacy Incident Reporting (CPhIR) Program is a voluntary program used by community pharmacies across Canada to analyze underlying process and system vulnerabilities that contribute to medication incidents in this healthcare sector, with the goal of preventing future incidents. Incidents involving methadone were analyzed to identify specific issues leading to problems with methadone therapy in the community.  Read more...
