July 22, 2015: New Toolkit for Healthcare Providers: Hospital to Home—Facilitating Medication Safety at Transitions
ISMP Canada recently developed a new resource for acute care: a toolkit to support the safe transition of patients and their medications from hospital to home. Hospital discharge is a critical interface of care when patients are at high risk of fragmented care, adverse drug events, and medication errors.
