July 21, 2015: Resources to Sustain Incident Learning - Ontario Critical Incident Learning Bulletin
ISMP Canada recently developed 2 NEW resources for acute care: a toolkit to support the safe transition of patients and their medications from hospital to home; and a checklist to assess the safety of labels for epidural medications. In addition, the Hospital Medication Safety Self-Assessment® (MSSA) is being updated to include learning from critical incidents reported through the Ontario Critical Incident Learning program. These resources are designed to support continuous quality improvement and may also assist hospitals as they prepare for accreditation.
