ISMP Canada, with support from Health Canada is hosting two separate full-day conferences for healthcare practitioners in the acute care and long
term care sectors. Sessions will be held in Kelowna, British Columbia on May 5 and 6, 2009.
Failure Mode and Effects Analysis workshops will also be offered concurrently.

Registration:
| Cost is $265 per day to attend a conference or workshop. A reduced rate of $475 will be offered to those who wish to attend both days. All costs include GST.

Attire:
| Business casual dress is suggested. You may wish to dress in layers as the room temperature may vary.

Advancing Safe Medication Practices in Acute Care Conference
This conference will focus on medication safety issues in the acute care environment and will highlight ISMP Canada projects, programs and tools
that can be used within your organization to enhance safe medication practices. The conference will feature presentations
by ISMP Canada staff and will include speakers from Western Canada. Join us for an opportunity to network with your colleagues and learn about
medication safety initiatives in your region and in other parts of Canada.
Preliminary Acute Care Conference Agenda
Failure Mode and Effect Analysis (FMEA) Workshop for Long Term Care
The ISMP Canada FMEA framework is a proactive, systematic, team-based approach that can be used to prevent process failures before they occur. FMEA focuses
on how and when a system or process will fail, not IF it will fail.
The workshop begins with an overview of the systems approach to error
management and the application of human factors engineering principles to healthcare environments. Participants spend most of the day in a facilitated,
interactive simulation exercise. At the end of the day, participants will have developed the expertise to serve on an FMEA team in a healthcare setting.

Failure Modes and Effect Analysis (FMEA) Workshop for Acute Care
The ISMP Canada FMEA workshop is a team-based, systematic and proactive approach identifying ways a process or design can fail, why it might fail,
the effects of the failure and how it can be made safer. At the end of the workshop you will have increased knowledge about the impact of system
factors on error potential, understand how human factors engineering applies to healthcare, and develop the expertise to serve on a FMEA team
in a healthcare setting.

- Who should attend the conference?
This conference is open to all healthcare practitioners. Participants may include, but are not limited to: CEOs and Home Administrators, Directors of Care, Chief Nursing Officers, Quality/Risk Managers,
Pharmacy Directors, Physicians, Staff Educators, Nurses, Pharmacists, Pharmacy Technicians and other interested healthcare practitioners.
- How much is the conference?
The conference fee is $265 per participant for one day or $475 for both days including GST.
- Can I attend a conference and a workshop?
Yes participants are invited to attend for two days at a reduced rate of $475 including GST. If you plan to attend a workshop, space is limited to 40 people per day so register early.
- Is there a discount if more than five individuals from the same institution want to attend?
Yes, a reduced rate is available to facilities who send five or more individuals. Multiple registrations must be submitted together
to qualify for the reduced rate. For more information email conferences@ismp-canada.org.
- How do I register more than one participant from our organization?
Fill out a separate registration form for each individual and mail or fax with payment
to ISMP Canada. Provide one cheque or credit card payment for all registrants from the same organization.
- How do I pay for the conference?
Payment can be made by cheque or credit card (Visa and MasterCard only) or PayPal.
All payments by cheque must be mailed with the registration forms.
To pay by credit card, complete the electronic registration form, or print and fax the pdf registration form.
- Will I receive a receipt for payment?
Yes, ISMP Canada will issue a receipt by email once payment is received in full.
- How will I know that my registration has been accepted?
You will receive a confirmation email from ISMP Canada within 10 days indicating that your registration has been processed and payment received.
- What time should I arrive to register?
On-site registration will begin at 8:00 am. Early arrival will ensure a smooth registration process.
