Accreditation Canada Readiness  - Do You Know Where You Are?
A New “GPS” (Global Practice Status) tracking tool can help!

This one hour webinar is designed to provide hospital administrators, medication safety, and quality officers with a practical example of a mechanism in use within a Canadian region that facilitates the tracking of the status and the progress of Accreditation Canada Required Organizational Practices (ROPs) across multiple units and sites.

Using examples of Accreditation Canada ROPs as a guideline, the session will walk participants through the use of the Fraser Health data collection tool. The demonstration will include how the data is obtained from multiple sites and how the display of this information in a “dashboard-style” that provides an overview of the ROPs and their status across multiple sites supports a more focused approach of efforts to achieve compliance with ROPs.

Key Messages:

  • simplify data collection,
  • inter-rater variability can complicate objective assessment and reporting of progress, and
  • use a tool with standardized definitions and rating scales to mitigate potential variation and to enable meaningful roll-up and interpretation of the data.

Learning Objectives:

On completion of this webinar, participants will be able to identify:

  • the principles needed to maximize utilization of a data collection tool,
  • the different levels of analysis that can be achieved through use of the tool, and
  • how the use of a “dashboard-style” tracking tool can assist hospitals/health authorities to focus efforts to achieve compliance with ROPs.

Audience: Hospital administrators, medication safety and quality officers, and healthcare practitioners (e.g., nurses, pharmacists, pharmacy technicians, and physicians) interested in medication safety.

Cancellation Policy
Notice of all cancellations must be provided in writing at least 7 days prior to the event. A $75 administration fee will apply to all refunds. There are no refunds for registrants who cancel after the cancellation deadline.

Registrants may postpone attendance to a future workshop however a $75 administration fee will apply if notice of postponement is received less than 7 days prior to the event.

Mits Miyata - Pharmacy Director
Lower Mainland Pharmacy Services, British Columbia

Mits Miyata is a Pharmacy Director for the Lower Mainland Pharmacy Services. Responsibilities have included the portfolio of Medication Safety along with Accreditation. He has been actively involved in the profession of Pharmacy including Pharmacy Examining Board of Canada, College of Pharmacists of BC, BC Ministry of Health, and the Canadian Society of Hospital Pharmacists.

Mits’ keen interest in the measurement and application of standards has led him to develop a tracking tool to address the challenges of tracking Accreditation Canada ROPs.

 

Janice Munroe - Pharmacy Medication Safety Coordinator
Lower Mainland Pharmacy Services, British Columbia

Janice Munroe is the Pharmacy Medication Safety Coordinator for the Lower Mainland Pharmacy Services with a keen interest in system and practice changes that improve patient, client, and resident safety. She is responsible for the portfolio of Medication Safety including Accreditation.

Since 2008, Janice has been an active Accreditation Canada surveyor. Most recently, she was invited to join Accreditation Canada’s Managing Medications Working Group which is responsible for reviewing the Qmentum standards.

Janice has been actively involved in the profession of Pharmacy including the College of Pharmacists of BC, and the Canadian Society of Hospital Pharmacists.

Background

The Fraser Health Authority in British Columbia comprises 12 sites and 17 regional programs, with vast distances between sites. To facilitate data collection and analysis, the Fraser Health pharmacy department developed an in-house integrated “dashboard” tool to monitor accreditation-readiness for each of the medication-related ROPs across the health authority. This tracking tool enabled local sites to provide data based on standardized definitions. Linkages between components of the tool and strategies for “rolling up” the data were used to generate customized reports, which informed the assessment of accreditation-readiness and guided subsequent interventions. Fraser Health continues to refine and expand the tool to provide interpretable data across the Lower Mainland.
