Analyzing medication incidents one group at a time

This award-winning, one day interactive workshop presents the key concepts for conducting a multi-incident (aggregate) analysis, a technique designed to maximize efficiency by analyzing a group of medication incidents sharing a common topic.

During this workshop you will learn a step-wise process for conducting a multi-incident analysis, and situations where this methodology is most beneficial. You will have the opportunity for hands-on practice under the guidance of ISMP Canada experts. A variety of take home materials and tools will be provided, enabling you to confidently analyze your facility's medication incidents upon return to your home facility.

At the completion of this course, participants will be able to:

  1. Describe how multi-incident analysis complements other medication incident analysis strategies
  2. Describe the steps of a multi-incident analysis
  3. Apply the step-wise multi-incident approach to begin conducting analysis of small groups of medication incidents

Cancellation Policy
Notice of all cancellations must be provided in writing at least 7 days prior to the event. A $75 administration fee will apply to all refunds. There are no refunds for registrants who cancel after the cancellation deadline.

Registrants may postpone attendance to a future workshop however a $75 administration fee will apply if notice of postponement is received less than 7 days prior to the event.

ISMP Canada reserves the right to cancel or reschedule workshops. In the event of a cancellation, fees paid will be credited towards a future workshop, or fully refunded.

On entry to the building, check in with Security to obtain a visitor ID badge (next to the lobby elevators).

We are located in the Procter & Gamble Building - 4711 Yonge Street - North of the 401 on the East side of Yonge Street, South of Sheppard Avenue.

Driving Directions:

Note that parking is NOT available in the Proctor and Gamble building. Indoor paid parking is available at the Sheppard Centre (Yonge and Sheppard). Outdoor paid parking is available on the southwest corner of Yonge and Sheppard (1 block north of the office) and on the north side of Avondale Ave, just east of Yonge Street (2 blocks south of the office).

Transit:

We are located on the "Yonge Line" - please get off at the Sheppard Station. Exit the station at the south end of the platform - "Poyntz Avenue". (Tip: The last car of the Subway, when it is heading North, will put you at the "Poyntz Avenue" exit.) Proceed through the turnstile, into the Procter & Gamble building, up the escalator to the Security Desk, and get a Visitor's Pass.

If you return the same way, you must have tokens or a MetroPass, as there are only turnstiles when you leave through the Procter & Gamble building - no TTC representative is on duty. If you only have tickets, then it is a short walk outside up Yonge Street to the North entrance of the Sheppard Station (Sheppard Centre), where there are TTC attendants.
