Using the BPMH to reduce medication errors in home healthcare

How medication reconciliation adds value in community based patient care

Program Abstract

This one-day workshop is designed to provide home health professionals with an overview of transition-related medication incidents and how medication reconciliation (MedRec) adds value in community based patient care.

There will be a specific focus on the importance of the Best Possible Medication History (BPMH) within the medication reconciliation process. Participants will learn how to conduct a BPMH interview and document the necessary components of a BPMH, and how to conduct a systematic review to enhance the safety of transitions to home from other levels of care.

Learning Objectives

On completion of the workshop, participants will be able to:

  1. Describe common transition-related medication errors and adverse drug events in the home
  2. Understand how MedRec enhances safety for patients
  3. Understand medication nomenclature
  4. Confidently perform the systematic process of collecting a BPMH
  5. Develop awareness of the roles of various health team members in resolving medication discrepancies
  6. Partner with patients to improve medication safety
