April 19, 2013: Institute for Safe Medication Practices Canada Partners with Health Quality Ontario (HQO)

The new strategic partnership between ISMP Canada and HQO will help to address preventable adverse events by increasing uptake, quality and consistency of medication reconciliation (commonly referred to as MedRec) during patient transitions of care within the health care system.

MedRec, an element of medication management, is a formal process in which health care providers work together with patients, families and caregivers to ensure that accurate and comprehensive medication information is communicated consistently among all relevant health care providers (e.g., primary care, pharmacy, home care, etc.).

ISMP Canada’s partnership with HQO will promote province wide enhancement of system capacity, standardization and spread of MedRec and safe medication practices.

The partnership will leverage ISMP Canada’s current best practices, processes, tools and content to enhance practice and build capacity by complementing HQO’s existing programs such as bestPATH and Quality Improvement Plans.

Health Quality Ontario is an independent crown agency funded by the Government of Ontario through the Ministry of Health and Long-Term Care. HQO reports directly to Ontarians on access to publicly funded health services, health human resources in publicly funded health services, consumer and population health status, and health system outcomes.
