Patients are at high risk of medication errors due to fragmented care at transitions. In collaboration with the Canadian Patient Safety Institute, Patients for Patient Safety Canada, the Canadian Society of Hospital Pharmacists, and the Canadian Pharmacists Association, ISMP Canada developed '5 questions to ask' to help consumers and their caregivers start a conversation about medications with their healthcare provider at transitions in care.To learn more visit:
