March 30, 2016: Hospital Medication Safety Self Assessment (MSSA) – Launch of Canadian Version III, 2016

The Hospital Medication Safety Self-Assessment (MSSA), Canadian Version III (2016) was launched in March 2016. This revised version includes substantial new content from the Canadian Medication Incident Reporting and Prevention System (CMIRPS), the Ontario Critical Incident Learning (OCIL) program, and adapted content from the 2011 ISMP (US) Medication Safety Self-Assessment for Hospitals. Assessment items related to prevention of medication-related “never events”identified for Canadian hospitals in 2015 have also been included.

New/revised items: There are a total of 297 assessment items, including 69 new items and 77 revised items; 15 items have been deleted.

Alignment with Accreditation Canada Medication Management Standards and Required Organizational Practices: ISMP Canada and Accreditation Canada have complementary mandates to ensure patient safety, and many ISMP Canada recommendations have been incorporated into Accreditation standards. ISMP Canada worked with Accreditation Canada to indicate where assessment items within this document are aligned with the 2016 Qmentum Medication Management Standards (Version 10), including Required Organizational Practices.
