The Ontario Hospital Critical Incidents Related to Medications or IV Fluids Analysis Report is written annually based on critical incidents that are reported by acute care hospitals. The reporting of such incidents is the cornerstone for understanding vulnerabilities in medication-use processes and for building better and safer healthcare systems. ISMP Canada analyzes every critical incident that Ontario hospitals report to the NSIR, with the goal of preventing similar incidents through shared learning in our bulletins and annual reports.
