August 2007: TWO POSTIONS AVAILABLE: Full time medication safety specialist

LOCATION: ISMP Canada, Toronto ON

JOB RESPONSIBILITIES:
Working as part of an interdisciplinary team, the medication safety specialist will:

• Lead/support projects related to ISMP Canada deliverables in hospitals, long-term care facilities and community pharmacy, e.g.:
         medication reconciliation
         High alert medication safety projects
         Expansion of Medication Safety Self-Assessment use
         Environmental scans for selected drugs
  
• Participate in writing ISMP Canada safety bulletins
  
• Assist in the collection and analysis of medication incident data
  
• Participate in developing and sharing best practices and prevention programs
  
• Create/participate in educational programming for professional audiences
  
• Contribute to ISMP Canada collaborative activities
  
• Collaborate with practitioners, consumers, professional organizations, vendors, regulatory bodies and other stakeholders on a variety of medication safety topics
  

Other potential activities include:

• Perform Failure Mode and Effects Analysis (FMEA)
  
• Participate in Root Cause Analysis (RCA) of medication-related events
  
• Provide in-depth medication error analysis and problem solving
  

QUALIFICATIONS:

• Current licensure as a regulated health professional in a Canadian province
  
• Work experience in healthcare settings
  
• General familiarity with Canadian healthcare systems and processes
  
• Exceptional writing and editing skills
  
• Demonstrated leadership skills
  
• Demonstrated presentation skills
  
• Self-directed; project driven; ability to problem-solve
  
• Ability to work as a member of an interdisciplinary team
  
• Ability to travel (primarily in Canada)
  
• Knowledge of accreditation and professional practice standards is desirable
  
• Experience in medication safety or patient safety initiatives preferred
  
• Risk Management and Quality Improvement experience a plus
  
• Proficiency with Microsoft Office software
  
• Respectful of the strictly confidential nature of ISMP Canada work
