Several recent incidents, reported to ISMP Canada and shared in the media, have involved administration of expired vaccines to patients. Vaccine effectiveness cannot be guaranteed after the expiry date; therefore, following this type of incident, a replacement dose of the vaccine is often considered, subject to guidance from public health authorities.
