Understanding the hospital pharmacy technician’s role in the medication reconciliation process

1 day workshop
This one day workshop is designed to provide hospital pharmacy technicians with an overview of medication reconciliation.
There will be a specific focus on the importance of the Best Possible Medication History (BPMH) within the medication reconciliation process, and participants will learn how to conduct a BPMH interview and document the necessary components of a BPMH.
At the completion of this workshop, participants will be able to:
- Explain the role of medication reconciliation as it affects patient care
- Understand where and how medication reconciliation takes place
- Understand the role of the BPMH at all care transitions
- Describe the process of completing a BPMH
- Understand the benefits and limitations to the various sources of a patient’s medication information

Alice Watt, RPh, Medication Safety Specialist
Alice joined ISMP Canada in 2007 as a Medication Safety Specialist and received her BSc. (Pharm.) from the University of British Columbia. She is actively practicing in a community hospital as a clinical pharmacist and has over 15 years of experience in community and acute care settings.
Alice is currently involved in incident analysis, newsletter development and engaging consumers and health care practitioners in safe medication practices. In the past years, she has worked on pediatric opioid safety, the WHO High Fives: Action on Patient Safety Assuring Medication Accuracy at Transitions in care, and supported Safer HealthCare Now! medication reconciliation quality improvement initiatives.
Lindsay Yoo, BScPhm, RPh
Lindsay is a graduate from the University of Waterloo School of Pharmacy. She joined ISMP Canada during her first co-op term in 2009 as a research analyst assistant, where she developed a strong interest in medication safety. Following this placement, she remained with ISMP Canada in the capacity of a consultant, and has been involved in a variety of medication safety initiatives. Currently, her main projects include coordinating and presenting at workshops, medication incident analysis, as well as community pharmacy-related initiatives. Lindsay has completed previous co-op terms and internship in hospital pharmacy, and is currently working as a pharmacist in the outpatient and community pharmacy setting.
