Behind the scenes: How medication incidents are analyzed and learning shared

This one hour webinar will focus on sharing information and learning from medication incidents to help improve patient safety and quality of care across Canada. Content will include:

  1. an introduction to the Canadian Medication Incident Reporting and Prevention System (CMIRPS)
  2. the use of case examples to describe and illustrate two complementary medication incident analysis strategies utilized by ISMP Canada:
    • Individual report analysis
    • Aggregate analysis
  3. an overview of strategies to identify contributing factors of medication incidents and systems-based recommendations for safe medication use.

Audience: Risk Management Staff, Nurses, Physicians, Pharmacists, Pharmacy Technicians, Pharmacy Students and other healthcare professionals.

Learning Objectives:
At the conclusion of this webinar, participants will be able to:

  1. Report medication incidents and near misses to the Canadian Medication Incident Reporting and Prevention System
  2. Understand the processes for conducting individual report analysis and aggregate analysis.
  3. Describe some strategies for identification of contributing factors of medication incidents and systems-based recommendations for safe medication use.

Roger joined ISMP Canada in 2007. His current responsibilities as a Research Analyst include conducting individual and aggregate analysis from the medication incidents in the ISMP Canada Medication incident database.

Roger holds a Bachelor of Science in Pharmacy degree (1999) and a Doctor of Pharmacy degree (2003) from the University of Toronto. Roger has worked in both community and hospital pharmacy settings, including staff pharmacist positions at Princess Margaret Hospital and York Central Hospital.


Patricia Hung, B.Sc.Phm. Candidate
Co-op Student, University of Waterloo, School of Pharmacy
Patricia is a student in the vanguard class at the University of Waterloo, School of Pharmacy. She has completed her second year of studies and is currently working at ISMP Canada as a Research Analyst Assistant for her co-op work term. In addition, she has experience in both community and hospital pharmacy across Canada in Alberta, British Columbia, and Ontario.

Notice of all cancellations must be provided in writing one week prior to the event. A $25 administration fee will apply to all refunds. There are no refunds for registrants who cancel after the cancellation deadline. ISMP Canada reserves the right to cancel or reschedule webinars.
