Ontario Strengthening Medication Safety in Long-Term Care Initiative

The Institute for Safe Medication Practices Canada (ISMP Canada) is partnering with 10 Champion long-term care homes in Ontario to improve medication safety and help address recommendations from the Justice Gillese Public Inquiry report. This initiative is funded by the Ministry of Long-Term Care and is designed to improve medication safety by providing support (tools, education and coaching) to homes. Read more ...

The Institute for Safe Medication Practices Canada (ISMP Canada) is pleased to jointly announce an important partnership with iMD Health Global (a CloudMD Software & Services company) to support education and increase awareness of the risks associated with medication errors. .:Canada's National Incident Data Repository for Community Pharmacies

ISMP Canada established a national incident data repository for community pharmacies through its community pharmacy incident reporting program. Community pharmacies in several provinces are already contributing to this national repository for continuous quality improvement, and pharmacies in other provinces are considering participation in this effort as well. The repository is helping to create a more cohesive information-sharing system that will facilitate better understanding of medication incidents and the development of more robust strategies to prevent harm.  Read more ...

     

.: Educational Support for Mandatory Reporting of Serious Adverse Drug Reactions and Medical Device Incidents

It will be mandatory for hospitals to report serious adverse drug reactions (serious ADRs) and medical device incidents (MDIs) to Health Canada, effective December 16, 2019. Four PowerPoint modules can be used (as entire modules or selected content) by hospitals, health care professionals, and educators, to explain, describe, or promote the reporting of serious ADRs and MDIs. Patients for Patient Safety Canada created an additional PowerPoint module to help patients and the public understand and promote the reporting of serious adverse drug reactions and medical device incidents.  Read more ...
