March 17, 2022: Safety Bulletin - National Poison Prevention Week (March 20 to 26, 2022)

Unintentional poisoning is the third leading cause of injury and death in Canada, with children being especially vulnerable to these preventable events. May 31, 2021: Safety Bulletin - Let's Talk with Kids about Their Medicine

5 Questions to Ask About My Medicine is a new pediatric-focused safety resource to help children and youth learn what questions they should ask when they receive medication from their health team. Involving children in their own care helps to give them a voice, so they can speak up when they have concerns. The handout was co-designed by children, caregivers, and providers to facilitate conversation during an appointment or patient consultation. Read more ...

May 31, 2021: Safety Bulletin - Structured Opioid Therapy

Structured Opioid Therapy is a team-based approach to opioid therapy that involves patients, prescribers (physicians and nurse practitioners), and pharmacists. It can be useful when the current opioid therapy is out of sync with the goals for function and pain control. A new Structured Opioid Therapy infographic has been designed by a collaborative team of researchers and health care providers from Sinai Health, the Centre for Addiction and Mental Health, the Saskatchewan Health Authority, and the Institute for Safe Medication Practices Canada, working alongside people with lived experiences. Read more ...

July 31, 2019: News Release: ISMP Canada supports Long-Term Care Public Inquiry Recommendations

News Release (Toronto)  –  ISMP Canada supports the recommendations released today in the Long-Term Care Public Inquiry Report. The focus of the report is to ensure improvements to the care and safety of long-term care residents are made; a goal that is consistent with ISMP Canada's commitment to advancing medication safety in all health care settings.

April 5, 2019: ISMP Canada CEO delivers successful Incident Analysis Workshop in New Brunswick

ISMP Canada President & CEO, Carolyn Hoffman, poses with Horizon Health Network workshop participants in Fredericton, New Brunswick. While in New Brunswick, Carolyn facilitated a two-day Incident Analysis Workshop for the Quality, Patient Safety and Risk Management staff.

"I had the pleasure of working with Alice Damon, Chief Quality & Patient Safety Officer and Lauza Saulnier, Regional Director, Quality and Patient Safety Services to prepare and deliver the workshop to meet the specific needs of their organization," said Carolyn. "I'm impressed by their systematic approach to building a strong and expert team of staff who will effectively support incident reporting, learning and sharing in the Horizon Health Network. ISMP Canada is pleased to play a role in this journey!"

September 24, 2018: Canadian Patient Safety Week: October 29 to November 2, 2018

ISMP Canada is collaborating with the Canadian Patient Safety Institute on this year's Canadian Patient Safety Week (CPSW) with the goal of reducing medication errors across Canada. The theme for this year's CPSW is Not All Meds Get Along – a prompt for patients and pharmacists to seek medication reviews for at-risk populations.  Read more ...
